Why does an anal fissure itch, and how do you calm it?

An anal fissure itches for four main reasons: the skin is regenerating as the tear heals, moisture or mucus irritates the area, a cream or wipe causes contact dermatitis, or the skin is being over-cleaned. Gentle washing with water, patting dry, a barrier cream and soft stools calm most cases; hydrocortisone never longer than 7 days.

In short

Why does an anal fissure itch?

Most people expect a fissure to hurt, not to itch, so the itching surprises them. In clinic we see four causes, often together:

  • Healing skin. As the tear closes, new, thin skin covers it. That regenerating tissue is irritable and itches, much like a scab elsewhere on the body.
  • Moisture. The anal canal produces mucus, the area sweats, and a sore anus is hard to clean perfectly. Damp skin softens, breaks down and itches.
  • Contact reactions. Creams, wipes, soaps and toilet paper with fragrance can irritate or sensitize the skin.
  • Over-cleaning. Scrubbing after every bowel movement, sometimes several times, strips the skin’s protective layer.

The pain of a fissure comes from the tear and the sphincter spasm, as we explain in Why does an anal fissure hurt for hours after a bowel movement?. The itching comes from the skin around the tear. That is why they are managed differently, and why a cream that relaxes the sphincter does not, by itself, stop the itch.

Is itching a sign that the fissure is healing?

Often, yes. A typical pattern is this: during the first weeks, the dominant symptom is sharp pain with bowel movements. As pain fades and bleeding becomes occasional, an itch appears in the same place. That shift usually means the tear is covering over.

But itching alone does not prove the fissure has healed. A chronic fissure can itch for months because its edges produce discharge. Use the other symptoms to judge progress:

What you noticeWhat it usually means
Less pain, less blood, new itchThe tear is closing; keep the treatment until the planned 6 to 8 weeks
Same pain and bleeding, plus itchThe fissure is still open and the skin around it is irritated
Itch with a damp, red, shiny areaMoisture dermatitis or a fungal infection, often after steroid creams
Itch that worsens after each application of a creamLikely contact reaction to that cream

Do not stop a sphincter-relaxing ointment because the pain is gone and only an itch remains. Stopping at that point is the most common reason a fissure reopens. The usual timeline is in Acute or chronic anal fissure? What changes after 6 to 8 weeks.

Can discharge or moisture make a fissure itch?

Moisture is probably the most underrated cause. A chronic fissure often has a sentinel pile, a small skin tag at its outer end, and sometimes an enlarged anal papilla at its inner end. Both create folds that trap mucus, sweat and microscopic traces of stool. The skin underneath stays damp for hours and itches. We describe the tag in detail in Sentinel pile: what is the lump next to a chronic anal fissure?.

Other sources of moisture:

  • Loose stools or diarrhea, which leave residue that is hard to clean without rubbing.
  • Coexisting hemorrhoids, which can produce mucus.
  • Heat and tight synthetic clothing, which increase sweat in the area.
  • Ointments applied in thick layers, which hold moisture against the skin instead of protecting it.

The fix is to keep the skin clean and dry, not sterile: water after the bowel movement, patting dry, a thin barrier layer and breathable cotton underwear. A small piece of dry cotton gauze between the buttocks can absorb moisture during the day; change it when it is damp.

Can fissure creams or wipes cause itching?

Yes, and it is common. Contact dermatitis around the anus looks like red, sometimes weepy skin that itches more every time the product is applied. Typical culprits:

  • Perfumed wet wipes and scented toilet paper. Fragrances and preservatives in wipes are well-known causes of perianal dermatitis.
  • Numbing creams. Local anesthetics can sensitize the skin with repeated use over weeks. Lidocaine is useful before bowel movements, but not as a cream applied many times a day; see Lidocaine cream for anal fissure: why it eases the pain but does not close the tear.
  • Hemorrhoid creams with several active ingredients, which add more possible irritants without treating the fissure.
  • The sphincter-relaxing ointment itself. Local itching is a recognized side effect of diltiazem and, less often, nifedipine. It is usually mild; a change of base or strength often solves it. Our guide on Side effects of anal fissure creams and how to manage them covers what to do.
  • Steroid creams used too long. They calm itching at first, but prolonged use thins the skin and favors fungal infection, which then itches more.

If the itch started or worsened after a new product, stop that product (not the prescribed ointment without talking to your doctor) and see whether the skin calms within a few days.

How should you clean the area without making it worse?

Hygiene is where most itching is created or solved. The goal is to remove residue without friction and leave the skin dry. The checklist below summarizes what we tell patients.

Checklist of what to do and what to avoid when an anal fissure itches: water and patting dry, zinc barrier, cotton underwear, no perfumed wipes, no scrubbing, no steroids beyond 7 days

A sitz bath of 10 to 15 minutes in warm, plain water after the bowel movement cleans the area and relaxes the sphincter at the same time; there is no need to add salts, soap or antiseptics. The details are in All guides. A handheld shower or bidet works too. Then pat dry with a soft towel or a hair dryer on the cool setting.

Soft, formed stools also matter for itching: they leave less residue and need less cleaning. That is one more reason to keep 25 to 30 g of fiber a day and enough water.

What can you put on an itchy fissure?

Avoid alcohol-based products, antiseptic solutions and home remedies with essential oils on broken skin. And try not to scratch: scratching damages the skin, the damaged skin itches more, and nails can reopen the edge of the tear. Short nails and cotton gloves at night help some people.

When is the itching not from the fissure?

Itching without the typical cutting pain during bowel movements is more often something else. The most frequent causes are pruritus ani (itching of the perianal skin from moisture, hygiene or diet), hemorrhoids, fungal infection, pinworms (especially in children, with itching at night), skin conditions such as psoriasis or eczema, and anal warts. Our comparison Fissure or hemorrhoids? The pain rule that tells them apart helps tell the two most common apart.

See a doctor without delay if the itching comes with:

  • Bleeding after the age of 45, or blood mixed into the stool.
  • Fever, pus or a painful swelling next to the anus.
  • A lump that grows, a sore that does not heal, or a fissure off the midline or several fissures.
  • A white, thickened or discolored patch of skin that does not go away.
  • Unintended weight loss or a change in bowel habit lasting weeks.

Persistent perianal itching that does not respond to hygiene and a barrier cream after 3 to 4 weeks deserves an examination; sometimes a small skin sample is needed.

Where does FISURIN fit?

FISURIN is a nifedipine and lidocaine ointment in a water-based hydrogel that our surgeons at Clínica Proctológica de Lima prescribe for chronic and post-operative fissures, with a written 6-week protocol. It targets the sphincter spasm and the pain, not the itch: it is not an anti-itch cream, and like any topical product it can occasionally irritate the skin, in which case your surgeon adjusts the plan. The protocol includes the hygiene and barrier steps described here. Today it is a magistral formula prepared under prescription in Peru and is not shipped abroad; FISURIN Hydrogel and FISURIN Relief are in preparation, with a waiting list. See what is inside FISURIN or how to get it where you live.

Frequently asked questions

Does an anal fissure itch when it is healing?

Often, yes. As new skin covers the tear, the area can itch while pain and bleeding fade. Keep the treatment until the planned 6 to 8 weeks; stopping when only the itch remains is a common reason for relapse.

Why does my fissure itch more at night?

Moisture and heat build up under the covers, and there are fewer distractions. Clean and pat dry before bed, apply a thin zinc barrier and wear cotton underwear. Night itching in children also raises the question of pinworms.

Can I use hydrocortisone cream for fissure itching?

For a short flare, yes, a 1 % cream once or twice a day, but never for more than 7 days. Longer use thins the skin, slows healing and favors fungal infection.

Should I use wet wipes if I have a fissure?

Better not, especially perfumed ones. Fragrances and preservatives in wipes are common causes of perianal dermatitis. Use water and pat dry instead.

Can my fissure ointment cause itching?

Yes. Local itching is a known side effect of diltiazem and, less often, nifedipine. It is usually mild. Do not stop the ointment on your own; ask your doctor about changing the base or strength.
